About The Position

The Workforce Group a LEMOINE company, is seeking a dedicated and compassionate Interim Social Worker to join our community -based multidisciplinary team. This role provides direct support and case management services to formerly homeless individuals, helping them achieve housing stability, access essential resources, and improve their overall well -being. This position requires a valid Social Worker, LPC, LMSW, or LCSW license. This is an interim position expected to last approximately 8–12 weeks. Location: Southeastern Louisiana, including but not limited to (Slidell, New Orleans, and Lafayette areas)

Requirements

  • Must hold a valid Social Worker, LPC, LMSW, or LCSW license.
  • Minimum of two (2) years of experience working with homeless populations, behavioral health, or social services.
  • The ideal candidate must have experience working with homeless populations, and be comfortable conducting home visits.
  • Experience providing case management, crisis intervention, and behavioral health assessments.
  • Strong understanding of the homeless service system, housing programs, and community resources.
  • The ability to build trusting relationships with vulnerable individuals and navigate complex social service systems.
  • Excellent communication, interpersonal, and crisis management skills.
  • Ability to work independently and collaboratively in a fast -paced environment.
  • Willingness to travel throughout Southeastern Louisiana as needed for service coordination and home visits.

Responsibilities

  • Conduct home visits to provide direct support, case management, and crisis intervention for individuals transitioning from homelessness to stable housing.
  • Develop and implement individualized service plans to help clients maintain housing, access healthcare, employment, and social services.
  • Assist clients in developing life skills, including financial management, healthcare navigation, and self -sufficiency strategies.
  • Provide clinical assessments and referrals for behavioral health, substance use, and medical services as needed.
  • Advocate for clients by coordinating with community agencies, social service providers, and healthcare professionals.
  • Maintain accurate and timely documentation of client progress, interactions, and service plans in compliance with program guidelines.
  • Facilitate group sessions and workshops focused on independent living, coping strategies, and mental health support.
  • Ensure adherence to trauma -informed care and person -centered approaches when engaging with clients.
  • Participate in multidisciplinary team meetings, case conferences, and training sessions to ensure coordinated care.
  • Perform other job -related duties as assigned.
