About The Position

Warbird Healthcare Advisors actively seeks experienced consultants to be considered for an Interim Director, Case Management role with a health system in Ohio. The successful candidate will provide overall direction for the clinical care coordination program which includes the coordination, negotiation, and procurement of care of patients to achieve quality and cost-effective patient outcomes throughout the continuum of care of the patient which includes all health care settings from acute inpatient treatment through follow up in the office, chronic disease management and preventative care.

Requirements

  • Must have a BSN; a master's degree is preferred.
  • A valid, unrestricted RN license from the Ohio Board of Nursing is required.
  • BCLS and ACLS certification is required prior to employment.
  • PALS and TNCC certification are required within six (6) months of employment; CEN is preferred.
  • Minimum of five (5) years of experience in case management, discharge planning or other cost/quality management program is required.
  • Minimum of three years' experience in a hospital clinical role is required.
  • Previous supervisory/management experience is strongly preferred.
  • Strong critical thinking, negotiation, and analytical skills are required.
  • Extensive knowledge of third-party payer systems and regulations related to patient placement in all levels of care is required.
  • Knowledge of leading practice in clinical care and payer requirements is required.
  • Strong analytical, data management and PC skills are required.
  • Excellent communication skills are required.
  • Demonstrated ability to work collaboratively with health care professionals at all levels to achieve established goals and improve quality outcomes is required.
  • Self-motivated with the ability to work both independently and as part of an interdisciplinary team.

Nice To Haves

  • CEN certification is preferred.

Responsibilities

  • Accountability for the management of human, fiscal, and physical/material resources of the Case Management department.
  • Responsible for budgeting, quality control, and oversight of those who provide case management and discharge planning services.
  • Ensure initiatives are in place to assure quality, cost containment, patient and physician satisfaction, and associate engagement goals are met.
  • Facilitate the performance improvement process to improve care transitions, reduce readmissions, improve throughput and maintain high levels of professional practice.
  • Integrate findings into interdisciplinary hospital activities to effect change.
  • Collaborate with other disciplines in case review to identify opportunities for improving patient care outcomes, reduction of future risk, and enhancing customer satisfaction.
  • Provide resource and consultation to senior leadership regarding care coordination and regulatory standards.
  • Promote innovation, team building, and processes that enhance work environment.
  • Utilize and disseminate research and best practices recommendations to enhance evidence-based practice and autonomous decision making.
  • Implement, monitor and measure both clinical and financial success upon approval.
  • Serve as a resource to staff, interpreting philosophy, goals, standards, policies and procedures.
  • Collaborate with other leaders to create and implement an operational plan that aligns with the system's strategic plan.
