About The Position

We are seeking a full-time Intensive In-Home Therapist provide community-based/in-home individual and family therapy for children, youth, adolescents, adults and their families. The person in this position will work with clients and families/natural supports to assess their strengths and needs, assisting families with planning, developing, implementing, and assessing treatment and ancillary services, recommending the types of treatment modalities needed and providing these services directly or by referral. Requirements

A valid driver’s license and current automobile insurance is also required.
At a minimum, a Master's Degree in counseling, social work, or psychology and 1 year experience in mental health issues, or an individual with a current license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession.
LCSW/A, LCMHC/A or LCAS/A

Responsibilities

Serve as a member of the client’s clinical treatment team and create, develop, monitor, revise, and implement the client’s Person-Centered Plan.
Participate in the development of crisis plans and transition plans when clients move in and out of services.
Provide individual and family focused support to clients and their families/natural supports based on their needs, diagnoses and best evidence-based practices.
Provide any written documentation as required. This will include treatment notes, goals and plans for the medical record (e.g., Service notes, person-centered plan etc.)
Work to implement therapeutic interventions, crisis interventions, behavioral interventions, psycho-educational activities and other best evidence-based practice therapies.
Work with clients, families, natural supports and collateral agencies in reviewing, applying and achieving goals as indicated in the Person-Centered Plan.
Contribute to and participate in child and family team meetings.
Provide parent/guardian/natural support training that will aid persons interacting with the client in dealing more effectively with the client.
Assist in all service delivery as formally planned with the clinical team.
