About The Position

At Merakey, we put heart and soul into everything we do! We are seeking a full-time Intensive Case Manager to join our Adult Behavioral Health team based out of Carlisle, PA. The Intensive Case Manager provides high-quality, community-based mental health services to adults living in Cumberland and Perry Counties. This role focuses on reducing hospitalizations, promoting independence, strengthening support systems, and supporting vocational and educational goals.

Requirements

  • Bachelor’s degree in sociology, social work, psychology, or related field; OR high school diploma/GED with 12 college credit hours completed in relevant coursework, plus two years’ experience in mental health; OR a registered nurse (RN) license.
  • Must have an active driver's license and reliable transportation during working hours.
  • One year of experience working with adults in public or private human services is preferred.

Responsibilities

  • Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
  • Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
  • Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
  • Support clients in accessing psychiatric care, community resources, and social supports.
  • Provide education and support to families and significant others.
  • Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
  • Participate in clinical supervision, staff meetings, and quality improvement initiatives.
  • Participate in on-call rotation for caseload.
