Under general supervision, an employee in this class provides intensive case management services for clients in a mental health setting. The work is concerned primarily with planning and problem-solving to overcome obstacles faced by clients such as systems rigidity, fragmented services, under-utilization of services, and lack of accessibility to certain services and resources. Supervision is received from a professional superior through review of case records and reports, and individual and staff conferences. Does related work as required. Key Elements Of The Role : Responsible for helping youth and their family access needed services; including service planning, child and family team meetings, referrals and access to needed services, advocacy, and coordinate and manage services across system; Performs outreach services to engage youth; engage families and other natural supports to identify the youth’s needs, facilitate linkages and promote follow-through. Coordinate and evaluate a youth’s identified needs and provision of services, including prevention, wellness, medical, mental health, care transitions and social and community services; Develop and manage comprehensive, individualized care plans addressing medical, behavioral, educational and social needs, working collaboratively with each client and family; Maintain regular contact with clients and collateral contacts providing face-to-face visits in the home, school and community to provide crisis prevention/intervention/stabilization, referral and advocacy services, coordination of services and assistance with basic needs, as well as engage in discharge planning with youth and family; Maintain knowledge on available community resources, including but not limited to medical, behavioral health, housing, DSS entitlements, transportation, vocational and other services that address concrete needs; Maintain detailed, timely and accurate record keeping/charting, assessment forms, accountability forms and direct service requirements of state and county regulations Work as part of a care management team, attend and participate in regular team meetings to provide feedback and share resource information relating to client needs, issues and concerns.
