About The Position

The Integrated Team Based Care (ITBC) Older Adult Manager is responsible for providing clinical oversight, support, and professional development for assigned staff. This position is part of a matrixed leadership structure and is responsible for collaborating with ITBC leadership to create a robust system of support and accountability for all ITBC staff. This role works collaboratively with other Integrated Team-Based Care leadership to ensure staff meet program obligations including compliance with state licensing standards, county requirements, and internal policies. This role is also responsible for developing best practices, and serving as the subject matter expert for assessments, treatment, harm reduction, case presentations, and diagnostic methodologies. This position must understand and appreciate differences within our Cascadia community. As such, it is expected that this position promotes integrated care, our vision of trauma-informed and person-first approaches and helps create a work environment of safety and acceptance.

Requirements

  • Master’s Degree in a behavioral science field from an accredited college or university.
  • Must demonstrate the competencies as outlined in the Oregon Administrative Rule’s (OAR) to become credentialed as a Qualified Mental Health Professional (QMHP).
  • Minimum five years of experience in community mental health or related field; 3 years clinical experience providing community behavioral health services AND a minimum of 2 years staff supervisory experience.
  • Understand the unique clinical needs of individuals served with complex medical conditions.
  • Understanding of recovery model, motivational interviewing, illness management and recovery, evidence-based practices, and strength-based case management philosophies and strategies.
  • Understand and align with harm reduction philosophies, cultural responsiveness, and trauma informed care.
  • Operate computer hardware and software at a level needed to effectively perform job functions.
  • Ability to commute to other locations.
  • Ability to perform BLS, First Aid, and effectively use AED equipment.
  • BLS/ First Aid certifications are required and must remain current.

Responsibilities

  • In collaboration with leadership team set annual goals and objectives aligned with integrated treatment and the agency’s mission; participate in on-going review and revision of processes.
  • Assist with the development of an operating budget to support the programs goals and objectives.
  • Serve as Leader of the Day (LOD) weekly, handling client calls and crises.
  • Provide resources and tools to supervisees to ensure quality work.
  • Collaborate with leadership to develop clinical guidelines and operational workflows.
  • member of ITBC leadership, Provide clinical consultation to team as needed
  • Coordinate with the leadership team to offer one monthly clinical training for staff.
  • Advocate and collaborate with leadership teams, executive management, and other departments.
  • Attend both internal and external meetings to problem-solve, advocate for programs, and maintain positive working relationships with employees and stakeholders.
  • Oversee and leverage clinical data to monitor performance and assist in decision making.
  • Serve as a liaison between departments, facilitating collaboration among interdisciplinary teams, and communicating with stakeholders.
  • Comply with laws, regulations, policies, and procedures under which the program and company operates.
  • Comply with and manage relevant Contracts, Memorandum of Understanding (MOU), and Grants.
  • Assist in crisis situations as appropriate; follow protocols and procedures in coordination with the treatment team.
  • Respect and honor client rights and responsibilities and demonstrate professional boundaries and ethics.
  • Maintain accurate and up-to-date documentation that ensures compliance with all agency policies and procedures and local, state, and federal regulations, and billing requirements.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Understand safety regulations and evacuation procedures; participate in regular safety drills in compliance with program policies and procedures.
  • Complete assigned employee training in compliance with program and company requirements.
  • Provide guidance, support, and training to staff in alignment with company mission and values.
  • Participate in all scheduled staff meetings, supervision sessions, and other departmental and company meetings.
  • Provide regular supervision, coaching, and annual training oversight for direct reports.
  • In collaboration with program leadership, determine staffing requirements, recruit, hire, and train staff in a manner that encourages and maintains Cascadia’s Values.
  • Establish work plans and priorities for staff members, assess the progress of staff members.
  • Promote and complete annual employee training requirements on a timely basis.
  • Demonstrate commitment to continuing professional development.
  • Provide direction and coach staff in Standard Operating Procedures (SOP), Electronic Health Records (EHR), Trauma-Informed Care (TIC), Feedback Informed Treatment (FIT), evidence-based practices, community resources, crisis management, harm reduction, Oregon Administrative Rules (OAR), HIPAA, and mandatory reporting.
  • Participate in on-call rotation, providing after-hours support when necessary.
  • Take the initiative in thought leadership, innovation, and creativity.
  • In collaboration with leadership team develop, review and revise clinical guidelines and operational workflows; as well as set annual goals and objectives aligned with Cascadia’s mission.
  • Create, model, and maintain a work culture that supports Cascadia’s Values.
  • Attend and participate in all meetings as assigned, including but not limited to staff and team meetings, consultations with staff, individual and group supervision.
  • Develop and maintain strong relationships with internal and external customers to promote quality and efficiency in Cascadia operations.
  • Perform other duties as assigned.
