As part of the MHI team, the Integrated Health Therapist (IHT) serves as a provider and partner in total child wellbeing, based in a PCP's office, at a regional outpatient Hub, or a combination of the two based on program needs. The IHT conducts mental health services, including diagnostic assessment, triage, psychotherapy, and multidisciplinary team treatment planning in the provision of clinical intervention. The IHT applies principles of social work, psychology, counseling, and other behavioral health practices to offer services such as brief therapy, psychoeducation, case management, problem solving, advocacy, and crisis intervention. The IHT is connected to a patient/family primarily via referrals from the patient's PCP, which is facilitated by a Warm Hand-Off from the medical provider and followed by a same day assessment and triage by the IHT to identify and address mental health needs early. Post-assessment, the IHT provides brief, evidence-based therapy, and coordinates referrals, where appropriate. The IHT comanages patient care along the PCP and acts as the main contact for and liaison between the patient's referring provider, psychiatry team, care coordination team, and other treatment team members. The IHT supports medical providers through ongoing consultation and provides continuous mental and behavioral health education. Patient care is provided both in person and via telehealth depending on patient preferences and scheduling availability.

  • Conduct mental health services including diagnostic assessment, triage, psychotherapy, and multidisciplinary team treatment planning.
  • Apply principles of social work, psychology, counseling, and other behavioral health practices.
  • Provide brief therapy, psychoeducation, case management, problem solving, advocacy, and crisis intervention.
  • Facilitate Warm Hand-Offs from medical providers and conduct same day assessments and triage.
  • Coordinate referrals post-assessment and provide evidence-based therapy.
  • Comanage patient care with the PCP and act as a liaison between various treatment team members.
  • Support medical providers through ongoing consultation and education.
  • 2 Years of Experience.
  • Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) in the State of California.
  • Master's degree in social work, Marriage and Family Therapy, Counseling, Psychology, or a related field.
  • Successful completion of Rady Children's Medical Staff credentialing process.
  • Knowledge of a range of tools, methods, and treatments for mental health issues.
  • Experience in assessment and treatment of children and adolescents.
  • Knowledge and expertise in emotional, behavioral, developmental and psychology norms.
  • Strong ability to perform same-day triage and assessment.
  • Commitment to valuing diversity and contributing to an inclusive working environment.
  • 4 Years of Experience.
  • Previous experience in integrated mental health care settings.
  • Knowledge and expertise in school and individualized education plans/504 plans.
  • Experience in trauma, bereavement, chronic illness, catastrophic diagnosis, family violence, child protection, and bioethics issues.
  • Knowledge of evidence-based practice utilized in clinical interventions.
  • Experience in quality improvement, measurement-informed care, and outcomes.
  • Experience working in an acute-care hospital and/or health system setting.
  • Competitive salary range of $45.75 to $62.90.
