Requirements

  • Active Registered Nurse (RN) license in the state of Indiana
  • Willingness to obtain licensure in additional states if needed
  • Comfort providing telehealth services
  • Successful completion of background screening (criminal, driving, abuse/neglect, fingerprinting)
  • Valid driver’s license, acceptable driving record, and current auto insurance
  • Completion of required onboarding and training programs

Nice To Haves

  • Experience in behavioral health, substance use treatment, or community health
  • BCLS certification required (ACLS preferred)
  • Passion for whole-person, integrated care
  • Strong clinical judgment and critical thinking skills
  • Ability to work collaboratively in a multidisciplinary team
  • Excellent communication and patient education skills
  • Comfortable working with diverse populations and complex needs

Responsibilities

  • Perform comprehensive nursing assessments , including medical history, screenings, and vital signs
  • Monitor patient health status and identify medical needs during treatment
  • Develop and implement care plans in collaboration with the treatment team
  • Collaborate with physicians, advanced practice providers, and behavioral health clinicians
  • Coordinate medical care, referrals, and follow-up services
  • Advocate for patient access to necessary healthcare services
  • Administer and monitor medications, including injections and specialty treatments
  • Educate patients and families on medication adherence and health management
  • Monitor for side effects and communicate with providers regarding treatment adjustments
  • Provide education on disease prevention, risk reduction, and overall wellness
  • Support patients in understanding and managing chronic health conditions
  • Assist providers with exams, lab work, and special procedures
  • Monitor lab results and communicate findings to the care team
  • Complete accurate and timely documentation in the electronic health record
  • Respond to medical and behavioral health needs in real-time
  • Maintain professionalism and composure in crisis situations
