As an Integrated Health Manager, you will lead care delivery and clinical operations transformations that improve throughput, utilization, and frontline performance for providers and payers.

Requirements

• Minimum 5 years of consulting experience with healthcare payer and/or provider clients
• Minimum of 2 years of experience in population health, care management, or value‑based care programs
• Bachelor's Degree

Nice To Haves

• Experience supporting population health platforms or care management technologies
• Exposure to Medicaid, Medicare Advantage, or ACO operating models
• Background in health equity, community‑based care, or behavioral health integration
• Strong understanding of longitudinal and cross‑sector care models

Responsibilities

• Design and scale preventive, chronic, and complex care programs to close care gaps
• Lead care coordination and transitions‑of‑care initiatives across payer, provider, and community settings
• Support value‑based care strategy, operating model design, and performance management
