About The Position

This position supports the Organization in the following manner: The integrated care social worker position assists in providing mental health and SBIRT services to Heart of Ohio Family Health’s patients. They will work collaboratively with other departments, colleagues, and community agencies to provide high quality care to our patients.

Requirements

  • Must be Social Work Licensed in the State of Ohio (LSW/LISW)
  • Required education: Bachelor’s or master’s degree in related field
  • Knowledge of federal, state and local laws and regulations pertaining to Social Work Licenses
  • EMR/EHR experience required
  • Competency in working sensitively and respectfully with people of various cultures and social status
  • Skills to expertly deliver exceptional health care to the public

Nice To Haves

  • Experience with integrated behavioral health preferred.
  • Multi-lingual (Spanish, Haitian Creole) and/or experience in working with interpreters/diverse patient populations is preferred.
  • Ability to successfully guide others to create an efficient, harmonious work environment

Responsibilities

  • Conduct SBIRT (screening, brief intervention, and referral to treatment) services to patients either in person or via telehealth.
  • Perform triage assessments and provide psychoeducation to patients referred to behavioral health services or experiencing a crisis.
  • Perform crisis intervention services including, but not limited to de-escalation, assessing for safety, brief counseling and psychosocial interventions, determining and referring to appropriate level of care
  • Collaborate with interdisciplinary team regarding patient care
  • Provide education to staff and providers regarding mental health and substance use disorders
  • Assist with coordinating internal and external behavioral health referrals, collaborate with referral agencies and resources as needed.
  • Provide short-term case management and connect patients with community resources.
  • If licensure includes -S, may be required to signs off on or support non-independent licensed social workers providing counseling services.
  • Accurate documentation in appropriate medical record
  • Perform the tasks of an LSW/LISW
  • Assist the providers as needed by administering the highest quality patient care in a sensitive and respectful manner
