About The Position
The Integrated Care Coordinator position is responsible for providing behavioral healthcare services, healthcare coordination, and conducting utilization reviews for eligible individuals. This role operates under the direction of the CSD Director and involves considerable independence in achieving program goals. The position encompasses three areas of assignment: Behavioral Health Agency Clinical Integrated Care, Behavioral Health Administrative Service Organization Integrated Care, and Utilization Review Integrated Care. The coordinator will engage in various tasks including counseling, crisis intervention, and collaboration with community partners to ensure effective care delivery.
