About The Position
The Integrated Care Coordinator (ICC) position is a vital role within the SSHARC/Cook County HIV Integrated Programs (CCHIP), aimed at assisting chronically complex HIV patients in addressing their specific medical and behavioral needs. Under the guidance of the SSHARC Site Team Leader (STL), the ICC will manage a comprehensive external referral system to provide client-centered, confidential services. This role is essential for linking clients with healthcare, clinical psychosocial, and supportive services, particularly for those living with HIV/AIDS who face challenges in accessing and adhering to healthcare services. The ICC will also offer treatment adherence counseling to ensure clients are prepared for and compliant with complex HIV/AIDS treatment regimens. In addition to direct client support, the ICC will collaborate with a multidisciplinary team to assist clients in obtaining necessary medical, social, community, legal, financial, and other non-medical case management services. This includes ensuring clients connect with core services such as dental, mental health, and substance abuse treatment. The ICC will also play a key role in the annual and six-month recertification processes for the Ryan White HIV/AIDS Program Eligibility (RWHAP), providing benefits counseling and assisting clients with enrollment and verification of their benefits. The ICC will supervise the SSHARC IDPH RIG 8 Prevention contractors and oversee program activities, working closely with the SSHARC multidisciplinary team and the EIS/Outreach team to coordinate community HIV testing and facilitate linkage and re-engagement to HIV primary care as necessary. This position requires a proactive approach to client management and a commitment to improving health outcomes for individuals living with HIV/AIDS.
