About The Position
The Integrated Care Coordinator for Psychiatry and Substance Use Disorder is responsible for providing comprehensive care coordination services to individuals with complex mental health and substance use disorders. This role involves assessing client needs, developing individualized care plans, facilitating access to services, and monitoring progress to ensure optimal outcomes. The Integrated Care Coordinator will work collaboratively with clients, families, providers, and community resources to deliver integrated and person-centered care.
Requirements
- Bachelor degree in human services field
- Minimum 1 year of experience in care coordination, case management or direct service with individuals with mental health and/or substance use disorder
- Valid New York State Drivers License
- BLS Certification
- Claxton Crisis Intervention Training

Responsibilities
- Identify patient strengths, needs, and barriers to recovery
- Develop individualized care plans in collaboration with clients, families, and treatment teams that incorporate evidence-based practices and recovery principles
- Coordinate access to a full range of services, including psychiatric care, substance abuse treatment, medical care, housing, employment, and social support
- Advocate for clients' rights and needs, ensuring they receive appropriate and timely care
- Provide crisis intervention and support, as needed
- Linking clients to emergency services when appropriate
- Monitor client progress and revise care plans as necessary to address changing needs and achieve desired outcomes
- Maintain timely and accurate documentation in the electronic medical record (EMR) in accordance with policy and regulatory requirements
- Participate in regular team meetings, case conferences, and supervision to ensure coordinated and effective care delivery
- Educate clients and families about available resources, treatment options, and self-management strategies
- Collaborate with community partners and stakeholders to develop and strengthen a comprehensive network of services
- Stay informed about current trends, best practices, and regulations in mental health and substance use disorder treatment
- Basic computer knowledge
