About The Position

The position serves as the first point of contact for new clients at the clinic, explaining available services and focusing on client engagement to access these services. The role involves completing health screenings, nursing assessments, and triaging healthcare needs for both new and ongoing clients. Participation in team meetings to develop care plans is essential, as is assisting clients in navigating the healthcare environment to ensure they receive appropriate care. The position also includes advocating on behalf of clients with healthcare providers and managed care organizations to ensure timely care, as well as assisting with coordination and access to services that address social determinants of healthcare outcomes. This includes providing transportation to healthcare appointments, identifying other needs that ensure access to healthcare, and assisting clients in obtaining access to housing and community resources. The role requires documenting tracking for appointments, medication, and other healthcare needs for assigned clients.

Requirements

  • Licensed Practical Nurse with current active license to practice in Kentucky.
  • Three years of nursing experience in a hospital, physician office, or behavioral care clinic.
  • Experience in community setting or integrated care environment.

Responsibilities

  • Provide first point of clinic contact for all new clients.

  • Assist with access to other community resources such as public assistance and financial benefits.
  • Develop knowledge base of other healthcare providers in the service area and collaborate as needed.
  • Document tracking for appointments, medication, and other healthcare needs for assigned clients.
