About The Position

The position serves as the first point of clinic contact for all new clients, explaining available services and engaging clients to access them. Responsibilities include completing health screenings, nursing assessments, and triaging healthcare needs for both new and ongoing clients. The role involves participating in team meetings to develop care plans, assisting clients in navigating the healthcare environment, and advocating on their behalf with healthcare providers and managed care organizations. The position also includes coordinating access to services that address social determinants of healthcare outcomes, providing transportation to appointments, identifying additional needs, and assisting with access to community resources. The role requires developing a knowledge base of other healthcare providers in the service area and collaborating as needed, as well as providing education and support for wellness activities. Additionally, the position assists clients transitioning from inpatient treatment, incarceration, or emergency department visits to outpatient care, ensuring a smooth transition and preventing readmissions. Collaboration with clients, families, and significant others to develop support networks is also a key responsibility.

Requirements

  • Licensed Practical Nurse with a current active license to practice in Kentucky.
  • Three years of nursing experience in a hospital, physician's office, or behavioral care clinic.
  • Experience in a community setting or integrated care environment.
