The position also includes coordinating access to services that address social determinants of healthcare outcomes, providing transportation to appointments, identifying additional needs, and assisting with access to community resources. The role requires developing a knowledge base of other healthcare providers in the service area and collaborating as needed, as well as providing education and support for wellness activities. Additionally, the position assists clients transitioning from inpatient treatment, incarceration, or emergency department visits to outpatient care, ensuring a smooth transition and preventing readmissions. Collaboration with clients, families, and significant others to develop support networks is also a key responsibility.
