About The Position

The position serves as the first point of contact for new clients at the clinic, explaining available services and engaging clients to access these services. The role involves completing health screenings, nursing assessments, and triaging healthcare needs for both new and ongoing clients. Participation in team meetings to develop care plans, assisting clients in navigating the healthcare environment, and advocating on their behalf with healthcare providers and managed care organizations are key responsibilities. The position also includes coordinating access to services that address social determinants of healthcare outcomes, providing transportation to appointments, and identifying other needs to ensure healthcare access. The role requires collaboration with clients, families, and significant others to develop support networks and provide education on wellness activities. Additionally, the position assists clients transitioning from inpatient treatment or other care settings to outpatient care, utilizing evidence-based transitional services to prevent readmissions.

Requirements

  • Licensed Practical Nurse with a current active license to practice in Kentucky.
  • Three years of nursing experience in a hospital, physician's office, or behavioral care clinic.
  • Experience in a community setting or an integrated care environment.

Responsibilities

  • Assists with access to community resources such as public assistance and financial benefits.
  • Develops knowledge of other healthcare providers in the service area.
  • Provides education and supports wellness activities.
  • Assists clients transitioning from inpatient treatment or other care settings to outpatient care.
  • Collaborates with clients, families, and significant others to develop support networks.
