About The Position

Pendleton Community Care, Inc. (PCC), a FQHC located in Franklin, WV, seeks a licensed behavioral health provider to staff its integrated primary care/behavioral health program based on the Primary Care Behavioral Health Consultation Model. Behavioral health providers co-locate with primary care providers. They will offer acute and short-term interventions using evidence-based behavioral interventions, psycho-education, counseling for health behavior change, case management, and consultation in the primary health care setting. PCC serves patients of all ages.

Requirements

• Minimum of a master's degree in social work, counseling, or psychology.
• Minimum professional license at the LCSW, LICSW, or LPC level.

Responsibilities

• Co-locate with primary care providers to deliver integrated care.
• Provide acute and short-term interventions using evidence-based behavioral interventions.
• Conduct psycho-education and counseling for health behavior change.
• Manage cases and provide consultation in the primary health care setting.
