Responsibilities

  • Assists primary care health providers with managing patients who use medical visits to obtain needed social support.
  • Efficiently moves patients into appropriate behavioral health specialty care, when indicated.
  • Utilizes the Collaborative Care Model (COCM) with primary care-based teams to administer care and a management of behavioral health patients.
  • Decreases unnecessary use of the Emergency Department.
  • Increases the adoption of life-style change regimens, e.g., smoking cessation, weight management, etc., to improve the health status of the MCH population.
  • Follows up with in person, telehealth, or telephone appointments where necessary.
  • Assists in in-patient consultation for behavioral health related issues and discharge if necessary.
  • Provides resources for patient continuation of care.
  • Assists primary care health providers in the recognition and treatment of mental disorders and psychosocial problems.
  • Assists in the early detection of 'at risk' patients, with the aim of preventing further psychological or physical deterioration.
  • Assists the health care provider in preventing relapse or morbidity in conditions that tend to recur over time.
  • Records patient's medical record and updates medical history as necessary.
  • Completes Social Determinants of Health (SDoH) screenings.
  • Provides education to patients to assist in preventing and managing addiction to pain medicine or other controlled substances.
  • Offers instruction to patients concerning the prevention and management of functional disabilities.
  • Helps health care providers obtain quality clinical outcomes with high prevalence mental disorders.
  • Helps primary care health providers treat and manage patients with chronic emotional and/or health problems efficiently and effectively.
  • Improves the quality of PCP interventions with warm handoffs utilizing the Collaborative Care Program (COCM).
  • Fosters a team-based approach with primary care peers focused on best outcomes for patient care.
