About The Position

The Integrated Behavioral Health Consultant (BHC) plays a crucial role in the primary care treatment team, focusing on identifying, triaging, and managing patients with both medical and behavioral health issues. This position involves delivering integrated clinical services, collaborating with medical staff, and providing psychoeducation and targeted behavioral interventions to enhance patient care within a community health center setting.

Requirements

  • Master's Degree or higher in Psychology, Social Work, or related field.
  • Licensed in the State of Minnesota as a mental health provider (LPC, LPCC, LGSW, LICSW, MFT, LMFT).
  • Experience providing care, preferably in a community health center setting.
  • Bilingual in English/Spanish preferred but not required.
  • Demonstrated success in working effectively with target population(s).
  • Ability to operate in a fast-paced, continuously evolving environment.

Nice To Haves

  • Experience working in a multi-site environment is highly desired.
  • Knowledge of Psychopharmacology.
  • Proficient with Microsoft Office Suite or related software.

Responsibilities

  • Deliver innovative integrated clinical services within the Primary Care Behavioral Health (PCBH) model.
  • Engage in collaboration with medical providers, medical assistants, and nurses to deliver holistic patient care.
  • Identify and connect with patients who would benefit from behavioral interventions.
  • Provide personalized individual, couples, and group sessions.
  • Perform comprehensive screenings, assessments, and make precise referrals to specialty clinical services.
  • Provide expert consultation and coordinate care with primary care staff.
  • Champion patient advocacy for those requiring specialty behavioral health services.
  • Respond to psychiatric emergencies and crises during clinical hours.
  • Focus on impactful case management and care coordination.
  • Foster community connections to elevate patient care and community health.
