About The Position

With the goal of access, quality, efficiencies, and patient satisfaction, the Integrated Behavioral Health Consultant - Health Steps Specialist provides evidence based episodic services and clinical interventions that targets the emotional, cognitive, behavioral, situational, and contextual barriers to the patient's physical health and wellness to a high volume of patients for a variety of emotional and psychiatric problems through an integrated and comprehensive team-based model. As part of an integrated practice, they provide initial patient evaluations, triage assessments, crisis intervention management, brief individual counseling, skills training, care coordination, case management, resource management, advocacy, follow-up, and consultation to patients and providers on behavioral and mental health related issues. This position strives to remain accessible on a same day or next day basis as much as possible. They participate in interdisciplinary case conferences, work with primary care providers, and assist with program development. Supports the pediatric primary care team to provide interventions, referrals, and follow up for pediatric patients and their families, with a specialty focus on ages 0-3. The collaborative, team-based approach of this position contributes to the mission of HFHC. This position is full-time with a schedule of Monday through Friday 8am to 5pm.

Requirements

  • Education: Completion of of Master's in Social Work or Master's in Counseling program
  • Current California registration as Social Worker, Professional Clinical Counselor, or Marriage and Family Therapy
  • ASCW, APCC, or AMFT license (Level II)
  • LCSW, LPCC, or LMFT license (Level III)
  • Compensation & job duties vary based on licensure
  • NPI number, or obtain an NPI prior to employment start date

Nice To Haves

  • 3-5 years of clinical experience post master's degree with some experience in a medical setting
  • Experience and knowledge about early childhood growth and development, parent-child relational health, infant and early childhood mental health and family systems
  • Current Basic Cardiac Life Support (BCLS) certification for infants and adults. Must be obtained within 30 days of hire
  • Comprehensive knowledge of community resources, and public and private benefits programs.
  • Familiar with various funding sources such as Medi-Cal, Medi-Care, etc.
  • Experience mobilizing community resources, navigating through the healthcare continuum, and working with disparate populations.
  • Diagnosis and treatment of mental health problems commonly experienced by individuals experiencing homelessness and the uninsured.
  • Knowledge of psychosocial factors involved in administering health service for unhoused patients.

Responsibilities

  • Job duties & compensation vary based on licensure
  • Grant specific duties: conducts team-based well child visits before, during, or after primary care provider; conducts consultations and screenings for development, social-emotional functioning, relational health and anticipatory guidance; maintains child development support lines; maintains resource directory, provides individualized referrals, and tracks follow-up; engages in continued quality improvement to ensure the HealthySteps model is delivered with fidelity; participates in HealthySteps trainings, participates in reflective supervision; works closely with pediatric primary providers around care coordination, goal setting, coaching and education about key aspects of a child's development.
  • Carries out the goals of the clinic to provide high quality integrated care that is compassionate, culturally sensitive, equitable, and trauma-informed, to all patients and families who are eligible to receive services at HFHC.
  • Collaborates with care team members to provide care to an average of 5 patients per 8-hour workday, addressing behavioral health needs for all walk-ins. Prioritizes and provides on-demand visits in addition to pre-scheduled follow up visits.
  • Provides individual, family, and group Integrated Behavioral Health services to all HFHC service areas via in person or telehealth formats. Maintains a generalist approach, addressing the behavioral health needs of all clinic patients across the lifespan.

  • Conducts comprehensive intake and biopsychosocial assessments, including risk, psychosocial, and screening tools (PHQ-9, GAD-7, ACES, AUDIT, SBIRT, Columbia, etc.), to identify and respond to mental health, substance use, and social concerns which affect their health and well-being. Assists the primary care team with responding to positive screens (PHQ-9, GAD-7, AUDIT, etc.) through clinical assessment, intervention, and referral to specialty care when appropriate
  • Provides targeted assessment and evaluation, triage, brief intervention, referral, and acute crisis management.
  • Conducts comprehensive intake and biopsychosocial assessments, including risk, psychosocial, and screening tools (PHQ-9, GAD-7, ACES, AUDIT, SBIRT, Columbia, etc.), to identify and respond to mental health, substance use, and social concerns which affect their health and well-being. Assists the primary care team with responding to positive screens (PHQ-9, GAD-7, AUDIT, etc.) through clinical assessment, intervention, and referral to specialty care when appropriate
  • Develops, implements, and maintains individualized treatment plans for patients based on biopsychosocial assessment that identifies: symptoms, areas of life affected by symptoms, mental health diagnosis using DSM, medical necessity of treatment, patient centered goals and objectives, skills training, and other service needs. Completes discharge planning upon completion of treatment planUtilizes therapeutic modalities that match the fast-paced primary care environment (such as behavioral and cognitive modalities, value-based interventions such as ACT/FACT, motivational interviewing, etc.)
  • Facilitates specialized counseling groups or classes within the scope of license and training
  • Adheres to the applicable ethical, therapeutic, and legal standards, which govern service delivery, as well as policies, procedures and laws, which apply to the organization as appropriate
  • Works closely and collaborates with other Behavioral Health and HFHC staff to provide the best possible care within scope of practice and services. Consults with relevant care team members through education, support, and real-time consultation. Actively participates in the integrated care team setting by consulting and collaborating with all other service departments and staff including but not limited to Primary Care, Addiction Medicine, etc
  • Maintains good working relationship and communication with all staff, reflecting professionalism in all interactions
  • Maintains succinct, clinically appropriate, accurate, and timely clinical documentation in the EMR, ensuring encounter closures within 48 hours. Scans patient paperwork, obtains required signatures, and documents assessments, treatment plans, progress notes, and other required data in compliance with organizational and legal standards. Properly codes and documents all patient encounters to meet UDS, CQM, and HEDIS requirements
  • Provides ongoing training through indirect support and clinical expertise consultation to increase all team members' effectiveness and base of knowledge. Provides supervision and instruction of students as assigned as applicable
