About The Position

This full-time behavioral health clinician will work as part of a multi-disciplinary team, supporting the care teams and patients at our health centers. The clinician will provide case management and Brief Solution-Focused interventions for medically complex patients with co-morbidities, address issues of teen substance abuse, encourage smoking cessation, focus on patient engagement, behavioral change, treatment compliance, and health risk factor reduction, and assist with crisis assessment. Our ideal candidate will have a passion to improve the health for our most vulnerable patients by practicing our core values of respect, integrity, compassion, honesty, and community.

Requirements

  • Graduate degree in social work, Clinical Mental Health, or related field
  • Master’s level required
  • LCSW or LPC preferred
  • Minimum two years’ experience in the field of Behavioral Health working with an interdisciplinary team and low-income population

Responsibilities

  • Providing case management and Brief Solution-Focused interventions for medically complex patients with co-morbidities
  • Addressing issues of teen substance abuse
  • Encouraging smoking cessation
