About The Position

This full-time behavioral health clinician will work as part of a multi-disciplinary team, supporting the care teams and patients at our health centers. The clinician will provide case management and Brief Solution-Focused interventions for medically complex patients with co-morbidities, address issues of teen substance abuse, encourage smoking cessation, focus on patient engagement, behavioral change, treatment compliance, and health risk factor reduction, and assist with crisis assessment. Requirements

• Graduate degree in social work, Clinical Mental Health, or related field
• Master’s level required
• LCSW or LPC preferred
• Minimum two years’ experience in the field of Behavioral Health working with an interdisciplinary team and low-income population

Responsibilities

• Providing case management and Brief Solution-Focused interventions for medically complex patients with co-morbidities
• Addressing issues of teen substance abuse
• Encouraging smoking cessation
• Focusing on patient engagement, behavioral change, treatment compliance, and health risk factor reduction
• Assisting with crisis assessment
