The Social Worker, LICSW will function as a core member of a multidisciplinary team and communicate patient's health care needs with primary care, behavioral health services, health care facilities and/or payers. Acts as an advocate and helps patient gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. The Professional Counselor (Mental Health Counselor Associate) practices in a treatment setting under clinical supervision and is dedicated to evaluation, diagnosis and treatment of psychiatric and substance use disorders. The Professional Counselor (Mental Health Counselor Associate) will collaborate with patient/support system to evaluate and diagnose patients. The Licensed Professional Counselor (Mental Health Counselor) practices in a treatment setting under clinical supervision and is dedicated to evaluation, diagnosis and treatment of psychiatric and substance use disorders. The Licensed Professional Counselor (Mental Health Counselor) will collaborate with patient/support system to evaluate and diagnose patients. The Mental Health Therapist LMFT practices in a treatment setting under clinical supervision and is dedicated to evaluation, diagnosis and treatment of psychiatric and substance use disorders. The Mental Health Therapist LMFT will collaborate with patient/support system to evaluate and diagnose patients. Provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. The Social Worker incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The Social Worker will collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter-Multi-Disciplinary Team (IDT).
