About The Position

Requirements

  • Minimum: Hold a master’s degree in Social Work, Psychology, Marital and Family Therapy, or Clinical Mental Health Counseling.
  • Submits paperwork to the State of Oregon Professional Licensing Board (Social Work, Professional Counselor or MFT Associate) to become a registered associate within 30 days of hire and maintains Associate status with the licensing board.

  • Skills to intervene and promote reconciliation, compromise, and positive outcomes in difficult interactions.

Nice To Haves

  • Skilled at using electronic health records as applicable to area of work (EPIC)
  • Experience with psychosocial and functional assessments, individual therapy, short-term evidence-based counseling, and crisis intervention.
  • Experience working in the medical social work or community mental health field and knowledge of Integrated Behavioral Health in the community healthcare system.

Responsibilities

  • Participates in all aspects of Fellowship program including patient encounters, didactic and clinic rotations, evaluations, and special projects.
  • Works with mentors and preceptors to increase skills in clinical care.
  • Attends educational sessions to increase knowledge of topics such as integrated behavioral health, leadership, health equity, and professional skill development.
  • Participates in mentoring and didactics as assigned by behavioral health leadership to include being shadowed/observed in clinic with patients to support a sound grasp of the integrated model, effective collaboration and consultation across team members, and competence in delivery of evidence-based therapeutic interventions.
  • Develops understanding of care needs for underserved populations, with a focus on learning more broadly about medical and dental conditions that impact behavioral health as well as social determinants of health.
  • Serves patients of all ages for a variety of presenting care needs to include all health and behavioral health conditions. Develops care plans and provides evidence-based targeted interventions to support whole health care.
  • Collaborates with patients and care teams to provide support and increase knowledge and understanding of the impact of behavioral health issues on overall health outcomes.
  • Demonstrates competence in diagnostic and contextual assessments, treatment planning, case management, and consultation.
  • Participates in assessing, offering and/or referring to appropriate stepped levels behavioral health care to patients, collaborating with the PHLOW program at La Clinica for psychiatric medication management needs and referring to specialty mental health and substance use care when warranted.
  • Provides consultation to medical or dental providers and team on behavioral health needs to support overall patient goals and health outcomes.
  • Partners closely with Behavioral Health Support Specialist team members in care delivery and provides consultative support and clinical guidance when indicated
  • Adheres to team agreements and assists in any revisions as necessary.
  • Sees patients at primary clinic and other sites as assigned by La Clinica. Routinely shadowed by preceptors and mentors for observation and feedback on clinical growth.
  • Responds to the recommendations for new or modified approaches to patient treatment recommended from team-based clinical case conferences and/or peer review.
  • Documents billable services within 72 hours ensuring appropriate diagnostic and CPT coding.
  • Responsible for following workflows that result in capturing activities to meet patient engagement standards as outlined in the APCM program using patient touches to capture nonbillable care provided.
  • Obtains continuing education as necessary to meet requirements of licensure by the Oregon State Board of Social Work or Licensed Professional Counselors and Marriage & Family Therapists and maintains competency in the practice of the Behavioral Health Clinician.
  • Completes and maintains all privileging/credentialing requirements and standards established by La Clinica.
  • Maintaining regular, consistent, and predictable punctuality and attendance, including being available, accessible, and able to complete all required essential functions and assigned job tasks onsite as directed during your regularly assigned schedule or your assigned site(s) normal business hours.
  • Completes assigned trainings via La Clinica’s learning module system by required due date.
  • Other duties as assigned.
