Southwest Behavioral and Health Services is seeking a dedicated and empathetic Intake Specialist who can help us fulfill our mission of delivering compassionate care to enhance lives and improve communities. This position is a specialty role focused on working with adult individuals who have been released from incarceration and are transitioning back into the community. The Intake Specialist will be an integral part of a multi-disciplinary team within the Pathways Program, collaborating closely with the County Jail System to ensure a successful transition for individuals from jail services to community stability. The role requires effective communication and coordination with correctional and legal system staff, as well as direct contact with clients to provide necessary services. Responsibilities include case management, community outreach, service planning, crisis intervention, and behavioral health services. The Intake Specialist will work to facilitate connections to outpatient services, psychiatric medication appointments, and other necessary resources, ensuring that clients receive the support they need during this critical transition period. This position is based in Flagstaff, working Monday through Friday from 8 am to 5 pm, primarily out of the Pathways Program adjacent to the jail and the Flagstaff Outpatient Clinic. The pay for this role is $19.00 per hour, and a sign-on bonus of $1500 is offered upon hire, with additional bonuses available after 30 days and 6 months of employment.
