The Intake Specialist at Thresholds is a critical role within the Certified Community Behavioral Health Clinic (CCBHC) framework, responsible for managing the initial clinical record and making service level recommendations for all new referrals seeking services. This position involves conducting comprehensive assessments and evaluations to ensure that individuals receive the appropriate level of care tailored to their unique needs. The Intake Specialist will perform preliminary screenings, risk assessments, and detailed diagnostic evaluations, working closely with program leadership to ensure safety and effective treatment planning. In this role, the Intake Specialist will provide intake services, substance use screenings, and mental health assessments both on-site at the clinic and in various community settings, including referral residences, psychiatric hospitals, nursing homes, and public spaces. The position requires effective management of the assessment schedule and coordination of clinical services, ensuring seamless communication between treatment teams and other agency components. The Intake Specialist will also engage with collateral sources, such as collaborating agencies and inpatient hospital staff, to advocate for the members and maintain accurate records of all interactions as per agency protocols. Direct service provision is a key component of this role, which may include group counseling, case management, crisis intervention, and psycho-social rehabilitation services. The Intake Specialist may also be required to work flexible hours, including evenings and weekends, and provide transportation for program members when necessary. This position is essential in ensuring that individuals receive timely and appropriate mental health services, contributing to the overall mission of Thresholds to support community mental health.
