About The Position

The Intake Specialist in Behavioral Health Psychology at St. Elizabeth Physicians is responsible for providing patient care services through social work intervention. This role focuses on scheduling patients with appropriate Behavioral Health providers and offering resources to address social issues impacting their health. The Intake Specialist collaborates with the care team to enhance patient outcomes and ensure compliance with treatment regimens, while also creating a positive impression with patients and visitors.

Requirements

  • High school diploma or GED.
  • 2 years of experience in a community or non-community setting.
  • Expert knowledge of community, professional, and educational resources.
  • Knowledge of basic nutrition, behavioral factors, and psychosocial/family systems.
  • Knowledge of common safety hazards and precautions.
  • Knowledge of health promotions and patient education principles.
  • Ability to utilize computers for documentation and information access.
  • Proficiency in Microsoft Word and Excel.
  • Demonstrated ability to work in a team-based culture and independently with effective outcomes.
  • Experience in planning and coordinating multi-disciplinary communications.

Responsibilities

  • Comply with all applicable laws and regulations.
  • Provide patient assessment to ensure safety and proper provider placement.
  • Coordinate and optimize activities to develop high-risk patient outcomes, including thorough chart reviews and sharing information with assigned providers.
  • Educate patients by providing resources, timelines, and goals for improving their accountability in health programs.
  • Make appropriate referrals proactively.
  • Provide strategies for frequently admitted patients and those who frequently utilize the ED.
  • Respond to referrals for services within 3-5 business days.
  • Respond to voice messages within 1 business day.
  • Complete required documentation on all patient encounters and updates in the EMR.
  • Identify and document all Intake interventions.
  • Communicate only necessary information on charts in accordance with HIPAA guidelines.
  • Participate in peer reviews, EMR reviews, employee education, and quality assurance activities.
  • Complete other assigned duties in a timely and accurate manner.
  • Provide insight and guidance to other staff members regarding patient needs.
