About The Position

The position is responsible for the assessment of participants’ health and psychological needs, determining appropriateness for meal service, and coordinating and implementing service plans that include information, referrals, and linkage. The role involves direct interaction with participants and requires a comprehensive understanding of community resources and services.

Requirements

  • B.A./B.S. degree and work experience in the field of human services.
  • Computer and word processing skills.
  • Verbal and written communication skills.
  • Current and valid New York State driver's license and access to an automobile.
  • Capable of lifting a minimum 35 lbs.

Responsibilities

  • Initiate participant application, make home visits, assess appropriateness for service, obtain relevant information regarding social, health and psychological needs, if not available from another case management source.
  • Responsible for initiating home delivery service, refer to other appropriate resources, as needed, and provide continued follow-up, through serving of routes, visits, and phone calls.
  • Intake responsibilities include an initial call with the referral source or applicant, completion of intake form and referral to the appropriate Social Worker and/or other case manager.
  • All social workers are responsible to answer inquiries about home delivery service, provide back up and assistance to each other, as well as other case managers, work with appropriate staff to coordinate service delivery, and submit timely and accurate monthly statistical/activity reports and other reports as requested.
  • Participate in special projects, in-service training of Health Care/Social Agencies, staff and volunteers as required, work cooperatively with public and private community resources for the coordination of Agency services, and maintain required professional credentials.
  • Maintain current knowledge of community agencies and resources, ensuring dissemination of related information for all appropriate Agency personnel, and actively participate in community affairs related to Meals on Wheels programs and goals.
  • Assist with any and all mission work as needed and requested with some or no prior notice including, but not limited to: serving as a truck spotter, helping with Farm Market/Community Garden/Freight Farms/on and off-site food distributions, preparation of meals and serving home-delivered meals.
  • Other duties as assigned.
