We are immediately hiring for a FULL TIME Intake Coordinator This is an opportunity to work with a committed, creative, fun and fast growing team that is passionate about providing exceptional Community Based Services. Our services are provided in personal homes as well as independent and assisted living facilities. When you join us your key responsibilities will include: -Support the flow of home health and hospice admissions, promoting efficiency, outstanding customer services, and attention to detail -Data entry of referrals into the electronic medical record system -Verify insurance and obtain authorization as needed -Carefully review medical records to assure appropriate referral information is obtained -Perform telephonic outreach to physician offices to obtain complete referral information -Interface directly with patients to support the scheduling of home health and hospice assessments -Compile and report metrics from the admissions department -Work collaborative with the Home Health and Hospice Intake Coordinators, covering for these roles as needed Qualifications Eligible employees have the opportunity to enroll in a wide variety of benefit options.
