About The Position

The Intake/Outreach Coordinator at Golden Home Healthcare is responsible for establishing and maintaining positive relationships with customers, managing referrals, and driving new business growth. This role serves as the initial contact for prospective clients, assisting them throughout the waiver application process while also maintaining relationships with existing clients.

Requirements

  • Bilingual in Spanish and English.
  • Sales experience.

  • High school diploma or GED.
  • Valid Driver's License.
  • Experience with customer relationship management systems.
  • Strong computer skills.
  • Time management skills.

Responsibilities

  • Establish and maintain positive relationships with customers.
  • Manage referral processes and new business growth to meet goals.
  • Assist potential clients throughout the waiver application process.
  • Respond to inquiries from prospective patients.
  • Coordinate all referrals and travel to patient locations as needed.
  • Make and receive calls to/from physician offices, partners, and social workers.
  • Promote and educate clients about the services offered.
  • Understand and follow appropriate procedures and regulations related to home and community-based programs.
  • Develop professional relationships with outside agencies to assist incoming participants.
  • Coordinate and communicate effectively with office staff.
  • Contribute to a positive and productive company culture.
