About The Position

We are seeking a full-time Intake LPN to field all phone inquiries and take complete referrals for Home Health and Hospice services. The Intake nurse is a key position for building relationships in the community with our referral sources. Excellent customer service and communication skills are required. The function of the Intake LPN is to process referrals and verify insurances.

Requirements

• Current Pennsylvania LPN license
• Home health background preferred
• Strong interpersonal skills
• Positive, proactive attitude

Responsibilities

• Answers phones professionally
• Takes all necessary information on referrals to ensure all demographics, diagnoses, and medical orders are complete
• Makes initial determination if patient can be accepted based on insurance type, patient location, and ability to staff
• Collaborates with clinical team to ensure all accepted referrals are processed for admission and scheduling
