INTAKE COUNSELOR (DEGREED)-PRN WEEKEND 10 am to 6pm

Position Summary: Responsible for being able to perform all aspects of the hospital’s assessment and referral process, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health and/or chemical dependency status; making patient safety and level of care recommendations to the physicians(s); identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information. Requirements
- Master Degree required
- 1 to 3 years experience
Responsibilities
- Knows and understands the assessment and referral call system procedure.
- Follows Intake call sheet to resolution.
- Knows the Hospital services and is able to answer general service information questions, including admission and exclusion criteria.
- Completes assessment and evaluation of presenting patients.
- Informs patients/families of patient rights, responsibilities, and pertinent rules and regulations prior to admission.
- Follows EMTALA regulations and identifies high risk individuals presenting for emergency care.
- Effectively communicates information to physician for appropriate orders for services
- Initiates verification of insurance coverage to determine viability of insurance for individuals seeking admission.
- Performs other duties as assigned/required by this position.
