About The Position

The Intake Coordinator at Visiting Redi-Nurse is a crucial role for a self-managed and organized clinician (LPN) who is dedicated to providing high-quality nursing care. This position involves coordinating patient admissions, ensuring effective communication between various stakeholders, and promoting patient independence during their recovery process. The role can be part-time or full-time, with responsibilities that include reviewing service appropriateness, managing referrals, and coordinating case management with insurance providers.

Requirements

• Graduate of an accredited school of Licensed Practical Nursing.
• Current License as a LPN by the state of Florida.
• Some home health experience.
• Good typing/computer skills.
• Excellent communication skills.

Nice To Haves

• Preferred knowledge of Medicare Regulations.
• 1 year of nursing experience (preferred).

Responsibilities

• Review appropriateness of services with every referral.
• Utilize knowledge of regulatory (State, Federal and CMS) as it applies to the admission process.
• Maintain knowledge of clinical applications.
• Coordinate admission process by facilitating communication between physician, insurance companies, pharmacies, staff, and other facility services.
• Evaluate staffing in conjunction with nursing coordinators.
• Assign referrals as indicated and appropriate staffing level to referral.
• Assure that pending referrals are updated with written documentation.
• Respond to community information calls as needed.
• Coordinate case management with insurance clerk to assure insurance eligibility for services and pre-authorization of care.
