Why This Role Matters As an Intake Coordinator , you are the first step in a patient’s home health journey. Your attention to detail and coordination ensure patients transition smoothly from hospital or clinic to the comfort of home care. You will serve as the key connection between physicians, hospitals, families, insurance providers, and our clinical care team , helping patients receive the care they need — when they need it.
Requirements
- Home Health experience required
- Working knowledge of medical terminology and EMR systems
- Experience with WellSky/Kinnser strongly preferred
- Exceptional attention to detail and organizational skills
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively in a team environment
- Comfortable in a fast-paced, ever-changing healthcare setting
Nice To Haves
- Experience with insurance verification and authorizations
- Bilingual skills are a plus
Responsibilities
- Manage the entire intake process from referral to admission
- Collect and organize medical, insurance, and patient information accurately and efficiently
- Coordinate with hospitals, clinics, and physicians to ensure seamless patient transitions into home health care
- Review referrals and match patient needs with appropriate services, treatments, and equipment
- Verify insurance coverage and obtain necessary authorizations
- Maintain accurate electronic medical records and ensure documentation compliance
- Serve as a supportive point of contact for patients and families , answering questions and guiding them through the intake process
