About The Position

The Intake Coordinator will direct all daily patient referral and intake operations, including participating in the establishment and implementation of intake policies. This role involves initiating charts into the EMR system and maintaining workflow, as well as managing census, pending admissions, red flag, journey’s end, and care plan meetings on the stand-up board. The coordinator ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements. A key aspect of the position is establishing and maintaining positive working relationships with current and potential referral sources, building and monitoring community and customer perceptions of Heart to Heart Hospice as a high-quality provider. The role also includes gathering, collating, and reporting referral statistics, including key customer referral trends, and participating in strategic planning to identify opportunities for additional or improved services. The Intake Coordinator maintains comprehensive working knowledge of company contractual relationships to ensure patients are admitted according to contract provisions and participates in quality assessment performance improvement teams and activities. Additionally, the coordinator maintains comprehensive working knowledge of community resources to assist referral sources in accessing services not provided by Heart to Heart Hospice and ensures timely submission of all documentation.

Requirements

  • Degree in health care or related field preferred
  • At least three (3) to five (5) years of experience in health care, preferably in hospice operations
  • Admissions/Intake and customer service experience
  • Valid Driver’s License and Automobile Insurance
  • Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices
  • Experience with EMR software

Responsibilities

  • Participate in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs
  • Maintain comprehensive working knowledge of company contractual relationships and ensures that patients are admitted according to contract provisions
  • Participate in quality assessment performance improvement teams and activities
  • Maintain comprehensive working knowledge of community resources and assist referral sources in accessing community resources should services not be provided by Heart to Heart Hospice
  • Timely submission of all documentation
  • Performance of other duties as required
