About the position
The Intake Coordinator is responsible for facilitating the referral intake process and coordinating care among the interdisciplinary team, patients, their families, and referral sources. This role involves timely documentation of new referrals, verification of patient payor sources, and effective communication with various stakeholders to ensure quality care delivery.
Responsibilities
- Receive and document new referrals in a timely manner.
- Verify each patient's payor source and maintain insurance authorizations and initial certifications.
- Coordinate care with Nursing Supervisors, patients, and the interdisciplinary group.
- Communicate effectively with patients, staff members, healthcare professionals, and referral sources.
- Respond to agency and patient needs in a professional manner.
- Participate in agency-sponsored in-service training and QAPI program.
- Promote the agency's philosophy and administrative policies.
Requirements
- Minimum one year of experience in referral intake preferred.
- Experience with third party payers, Medicare, and Medicaid highly desirable.
- Medical field background preferred, including familiarity with medical terminology.
- Excellent computer and customer service skills.
- LVN/LPN preferred.
- Valid Driver's License and Auto Insurance required.
- HomeCare HomeBase experience preferred.
