The Intake Coordinator is responsible for processing patient referrals to determine establishing care, obtaining/reviewing financial and medical documentation needed for care, verifying insurance eligibility and benefits, obtaining insurance prior authorizations, and communicating with patient/family regarding coverage and payment responsibilities. Performs daily intake tasks and achieves KPI's according to Service Level Agreements (SLAs). Ensures referrals are processed timely and according to standard operating procedure (SOP).

  • Establishes and maintains relationships with referral sources by receiving and processing intake information for potential patients who require infusion therapy.
  • Collects information and performs an initial assessment to establish/update the patient.
  • Surveys and monitors referral sources.
  • Obtains initial patient information and enters/updates patient demographics in the EMR.
  • Verifies insurance benefits, obtains authorizations/recertifications, schedules delivery of pharmacy medication, supplies, and nursing service.
  • Requests necessary documentation from referring providers, patients, and their insurance companies.
  • HS Diploma/GED preferred
  • 3 years of related experience
