Requirements

  • Bachelor's degree in a related field
  • 2 years of work experience in a related job discipline.

Responsibilities

  • Referral Management - Track data on number of referrals, referral types and monthly available visit types for purpose of managing supply and demand.
  • Coordinate and maintain statistical data on number of calls and disposition and participates in Performance Improvement process for the department.
  • Ability to gather clinical, medical, and demographic data to determine appropriate referral and level of care.
  • Document information and refer patients to internal/external services as appropriate within timely manner.
  • Customer Service - Responds promptly to intake requests.
  • Initiates referral process in a manner to be regularly and conveniently available to meet the needs of CHMC's patients as determined by department management and the medical staff.
  • External Evaluation - Triage and evaluate referrals.
  • Assess for appropriateness and/or determine options within or outside of CCHMC.
  • Triage and evaluate the patient need vs. service availability of patients and families for outpatient referrals.
  • Advocates for the needs of the child and adolescent patient and family within the hospital and the community.
  • Patient Assessment - Assess patient need to determine phone call urgency.
  • If routine, assess needs and provide specific referral options.
  • If urgent, transfer to appropriate clinician.
  • Customer Service - Develops skills and knowledge to interact with third-party payers and managed care.
  • Maintain professional relationship with referral sources and managed care contacts.
  • Authorize outpatient care and collect specific benefit information.
