About The Position

Completes intake, processes referrals, and administers waitlist for prospective participants hoping to enter Samaritan House extended stay shelter programs for individuals and families experiencing homelessness. Provides outreach to referring partner agencies in the community to increase and streamline referral pathways. Provides auxiliary support to case management and programming teams, assisting participants in achieving self-sufficiency through support, advocacy, resource referrals and assistance.

  • Flexible and ability to multi-task in a fast-paced environment.
  • Capacity to identify participant barriers to activities of daily living.
  • Ability to use computer to enter participant’s information into case files and compile program statistics, fax machine, copier, telephone and motor vehicle.
  • Aptitude to assist participant in identified barriers.
  • Knowledge of the service population’s cultural and socioeconomic characteristics and the appropriate techniques and practices for the participant population.
  • Ability to interact effectively as a team member and independently with Agency staff and with a diverse participant base.
  • Completed Bachelor's degree (BA/BS) from accredited four-year college or university.
  • Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write simple correspondence. Ability to effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Responsibilities

  • Reviews referrals from community partners and assesses readiness for Samaritan House programs. Administers prescreens.
  • Runs and evaluates arrest records to determine eligibility for programs. Documents information and receipts for purchases in accordance with Accounting protocols.
  • Manages waitlist for Samaritan House extended stay programs, including follow up communication and directing waitlist questions.
  • Checks and verifies openings to determine number of intakes that can be scheduled. Coordinates bed and room moves, as applicable.
  • Schedules intakes in coordination with Social Case Workers. Coordinates with team to determine when additional intake slots are needed to meet demand.
  • Conducts intake and orientation of new participants, including completing required forms and assessments, setting expectations for shelter stay, and providing tours to new intakes.
  • Accurately and promptly completes all required documentation/data, including entering relevant participant information in daily logs before ending shift assignment.
  • Accurately enters and maintains enrollments in HMIS and ETO databases. Audits databases to ensure high quality of data. Develops and maintains appropriate records, statistics and documentation of services to the participants.
  • Assists participants in navigating on-site services including mental health services, case management, and programming.
  • May complete additional assessments, including, but not limited, to needs assessments and coordinated entry assessments.
  • Networks with community partner agencies to ensure accurate understanding of Catholic Charities Denver Shelters and how to refer clients.
  • Communicates effectively with staff across departments to resolve concerns and ensure all staff have appropriate information in a timely manner. Coordinates efforts with other departments to ensure high quality of care provided to the participants.
  • Enforces shelter guidelines uniformly and consistently. Follows procedures and guidelines set forth in operating guidelines manual(s) to help maintain structure and order in the living environment.
  • Assists in the development and implementation of intake processes and overall program structure that fosters positive participant experience and helps participants become more self-sufficient.
  • Applies appropriate techniques and standards in intake, program development, crisis de-escalation, and service delivery.
  • May assess and refer participants to the appropriate level of care within the continuum of Catholic Charities Shelter programs. May provide periodic visits to other Catholic Charities shelters to complete referrals and support staff in training regarding referral process.
  • Reflects Catholic Charities’ commitment to treating all persons with dignity and respect. Demonstrates skills and sensitivity to the cultural/linguistic differences of staff, clients and community.

  • Maintains confidentiality of participant and agency information.

  • Participates in staff trainings and programs as required.
  • May support with intake at other Catholic Charities Denver Shelter sites.
  • May support with case management and/or programming at Samaritan House as needed/desired.
  • Networks within the community to develop partnerships and identify high quality services for the participants.
  • Any other tasks or duties as assigned.
