About The Position

The Intake Specialist is a full-time, permanent position that provides central intake screenings and assessments, benefits assistance, and resource and referral services. This role is non-exempt and reports to the Tribal Wraparound Program Manager. The Intake Specialist administers all intake and assessment processes for Housing and provides benefits navigation. Responsibilities include managing admissions and referrals from various sources, assisting citizens with their housing needs, and ensuring a culturally sensitive atmosphere for clients seeking assistance. The position may require work during early mornings, evenings, and weekends, especially during the winter shelter season.

Requirements

  • An associate's degree in social work, Sociology, Criminal Justice, Human Services or related field is required.
  • Previous experience as a screener, intake worker, social worker, case manager, or care coordinator in a public or private agency is required.
  • Five or more years experience working with veterans required.
  • Experience working with Calfresh, Department of Rehabilitation, and Medicaid required.
  • Honesty, accountability, and transparency with a commitment to social justice.
  • Exemplary interpersonal skills with the ability to collaborate effectively with culturally diverse staff.
  • Excellent crisis de-escalation and communication skills are required.
  • Demonstrated intellectual independence and initiative.
  • Must be able to maintain a high degree of confidentiality concerning all matters about the Tribe.
  • Extensive knowledge and experience working with Native American communities, culture, customs and cultural law.

Responsibilities

  • Coordinates and handles the dissemination, receipt, review, and acceptance of all applications for DCR Housing programs and services.
  • Utilizes the Placement Criteria Assessment and Referral Guidelines to determine eligibility of services.
  • Works with Systems Partners in responding to referrals and follow-up.
  • Ensures pre-admission criteria are met prior to admittance or disbursement of funds.
  • Assists clients with benefit enrollment and accessing medical, dental, optical, psychiatric services, insurance, and other tribal and state programs.
  • Assists applicants in completing intake screening, which includes conducting applicant interviews.
  • Creates and maintains waiting lists and conducts annual application updates to determine applicant waiting list position.
  • Coordinates with Director, Administrator, and other staff on all potential intakes.
  • Refers patients who do not meet eligibility or appropriateness for services to another service provider or agency.
  • Provides Rapid Rehousing Services that includes emergency stabilization and coordinating with housing navigator and care coordinators.
  • Registers all clients in the electronic database system and maintains accurate information.
  • Works collaboratively with the grant manager, funder, administration on expenses and claims processing for grant-related funding.
  • Manages main housing phone line, maintains accurate call log, and communicates effectively with necessary parties.
  • Compiles required reports including grant reports and monthly department reports.
  • Compiles statistical data and reporting as needed.
  • Works with securing authorization for payment for services and referrals.
  • Establishes a provider list and ensures quality of such providers.
  • Ensures work orders are assigned to proper contractor/provider and payment is made timely.
  • Assists with scheduling and hosting of trainings for staff and coalition meetings.
  • Attends and actively participates in all team and staff meetings.
  • Ensures effective, professional communications within and outside of the Department.
  • Creates and maintains effective interpersonal relationships with other employees and clients.
  • Listens to tenant/client complaints and concerns and works to resolve them.
  • Ensures tenant/client confidentiality and demonstrates discretion.
  • Prepares daily log entries, incident reports, and other required paperwork.
  • Utilizes computer to record tenant/client information and prepare reports.
  • Maintains competencies in CPR, First Aid, MAP, and agency policies.
  • Develops and maintains skills through seminars and workshops.
  • May include transporting, travel, and facilitating meetings and seminars.
