About The Position

VNA Alliance is a faith based, non-profit organization based in Meadville, Pennsylvania. VNA Alliance serves Crawford, Mercer, Venango, Erie, Forest, Warren, Butler, and Lawrence counties. We offer skilled nursing, palliative care, and comprehensive therapy services. The Intake Assistant is responsible for supporting the clinical management team in the daily operations of the Home Health office. Contributes ideas and suggestions to improve techniques, methodologies, and productivity.

Requirements

  • Two (2) years of progressive experience in a home health setting preferred
  • Computer Skills: Microsoft Word and Excel
  • Excellent organizational skills with attention to detail
  • Good communication and interpersonal skills
  • Ability to prioritize and organize daily work load
  • Prior experience in scheduling and/or data entry preferred
  • High School education required

Responsibilities

  • Prepares and runs reports from the computer system and ensures information is forwarded to the proper individual.
  • Creates/updates/maintains spreadsheets.
  • Phone calls to patients to enhance patient experience.
  • Assists with Face-to-Face process.
  • Responsible for clearing faxes and distributing to correct individuals.
  • Assist with clerical duties associated with referrals
  • Enter patient demographic information into the database
  • Assist in statistical data gathering
  • Prepare weekly referral report
  • Other duties as assigned
  • Effectively communicates with co-workers, supervisors, patients, and all outside contacts in a professional manner.
  • Interacts with field staff, patients, families, physicians, other health disciplines, agencies, insurance companies, hospitals, labs, employers, government agencies and representatives as necessary
  • Demonstrates a high priority towards serving internal and external clients in a courteous and timely manner.
  • Promotes cooperation and understanding within the agency.
  • Maintains a professional standard of conduct, projecting a positive image of the homecare service line at all times.
  • Complies with risk management practices.
  • Adheres to all policies and practices of the company.
  • Maintains confidentiality of patient and staff information.
