The A&R Intake Assessment Clinician is responsible for: Assisting in the process of responding to referrals and other requests for services and information. Providing technical assistance to the discharge planning process, under the direction of the Director of A&R. Responsible for being the first contact with the community and therefore excels at providing resources, and support to callers and walk-ins. Providing general support to the case management program. Document and process inquiry calls according to Intake policies. Assess and stabilize patients before discharge or arrange hospital admission. Notify and assist the on-call doctor with patient interviews. Obtain administrative approval for all admissions. Complete admission paperwork following hospital policy. Maintain an intake log with all necessary information documented. Make follow-up calls to patients who fail to show up for appointments. Demonstrate guest relations by example and provide counseling to employees in need of training. Requirements
- Master’s Degree in a human relations field preferred.
- Must be licensed to practice in the State of Illinois as a mental health professional.
- Preferably one or more years’ experience in an intake/assessment setting within a behavioral health, human services department/ clinic or hospital.
- A minimum of two (2) years’ experience with customer service.
- A working knowledge of a Mental Status Exam.
- A working knowledge of the Addiction and Recovery field.
