About The Position

The Intake and Assessment Manager at LifeCare Alliance will lead a diverse team of staff, volunteers, and interns who conduct in-home and phone assessments to ensure customers receive the services they need. This role is pivotal in managing the Nutrition Programs intake and assessment workflow, ensuring that clients are accurately assessed for eligibility and receive appropriate services.

Requirements

• Associates or Bachelor’s degree in human services or social work
• Two years experience in a community services organization
• Minimum two years supervisory experience

Nice To Haves

• Experience working with older adults and handicapped individuals
• Experience in interviewing

Responsibilities

• Supervise and manage the Nutrition Programs intake and assessment workflow and staff.
• Lead and oversee the team responsible for interviewing potential and new home-delivered meal clients.
• Assess new and current clients to determine eligibility for LifeCare Alliance services.
• Ensure that all assessments are conducted in compliance with relevant laws, regulations, and guidelines pertaining to community health administration.
• Collaborate with community organizations and resources to enhance service delivery.
• Utilize effective management and supervisory practices to foster a productive and supportive team environment.
• Communicate effectively with diverse populations and audiences through speaking, meeting, writing, and corresponding.
• Maintain up-to-date knowledge of non-profit organizational dynamics and the needs of the elderly.
• Provide leadership and guidance to ensure the team meets organizational goals and client needs efficiently.
