Responsibilities
- Meets with families in their homes' at various hours of the day as well as on call duties as scheduled in potentially high-risk neighborhoods.
- Serves as a conduit of information between all parties to the case and identified stakeholders, including medical, educational and mental health providers.
- Conduct visits with children, parents and/or caregivers.
- Assess family needs and identify goals for family that support a behavioral change to meet the family's needs.
- Provide hands on education/support to families to meet identified needs.
- Identify and develop potential partnerships with community entities with access to priority population.
- Attends court hearings, prepares and files any required reports.
- A willingness to travel as position requires frequent travel; follows agency and state guidelines regarding driving, rules of the road, and safety.
Requirements
- Bachelor's Degree in Social Work or related human services field.
- 0-6 months case management experience preferred but not required.
- Life Coach Certification, Child Welfare Employee License preferred but not required.
