About The Position

We are looking for nurses to join our team! The role will entail performing initial and ongoing patient/family assessments, including physical, emotional, psycho-social, cultural, and spiritual evaluations to ensure and coordinate the delivery of comprehensive, holistic services by the Interdisciplinary Hospice Team. You will provide palliative nursing care and symptom control in accordance with physician orders and the IDG plan of care. Documentation and entry into EMR of all assessments and interventions must be completed before the end of the shift. You will develop and prioritize the interdisciplinary care plan goals of the patient/family, hospice team, and attending physician prior to team meetings. Additionally, you will provide anticipatory guidance to the patient/family to assist in preparing for coping with events as they occur. Effective communication in English, both verbally and in writing, is essential. You will also revise nursing diagnosis, goals, interventions, expected outcomes, and the plan of care based on assessment data and input from patients and family members prior to the weekly IDG meeting.

Requirements

  • Graduate of an accredited nursing school.
  • Valid New York State nursing license.
  • Minimum of at least 2 years of nursing experience required.
  • Minimum of at least 1 year ICU, Med-Surg, ER or similar required; one year of hospice experience preferred.
  • Venipuncture and IV placement certification preferred.
  • Ability to exercise critical thinking skills.
  • Skill to prioritize and handle multiple tasks effectively.

Nice To Haves

  • Certification in Hospice and Palliative Care.
