About The Position
The social worker at Altru Hospital plays a crucial role in addressing the psychosocial needs of patients and their families. This position involves screening and assessing these needs, identifying discharge planning requirements, and providing essential information and referrals to community resources and services. The social worker will engage in supportive counseling, participate in interdisciplinary team staffing, and serve as a resource for patients, families, staff, and physicians by communicating vital information regarding the patient's psychosocial and discharge status. In addition to these responsibilities, the social worker will document assessments, interventions, and plans in accordance with JCAHO standards. They will also identify discharge planning and teaching needs upon patient admission, assist in developing and coordinating discharge plans, and arrange post-discharge services tailored to patient needs. Monitoring and evaluating the effectiveness of the case management program is also a key responsibility, along with participating in continuous improvement efforts. The social worker will care for patients across all age groups, from infancy to geriatric populations, ensuring comprehensive support throughout the continuum of care.
