The Social Worker provides direct clinical care to patients in the therapeutic process from admission to discharge, and provides individual, family, and group therapy, preparing discharge plans as indicated. The Social Worker will interface with internal and external stakeholders to ensure patient-centered, evidence-based, high-quality care for patients. Duties/Responsibilities: Carries a clinical caseload, conducts family therapeutic meetings utilizing biological, psychological, and systemic perspectives in understanding patients and their families to develop treatment plans and interventions from those perspectives appropriate to the ages of patients served. Works cohesively with other team members and is a contributor to the team; actively communicating with the Director of Social Work and other Social Workers, as needed. Ensures care provided is appropriate to the ages of the patients served. Holds self-accountable to helping the patient and family achieve desired results, understanding and identifying professional strengths, limitations, and challenges. Actively participates in clinical rounds & social work staff meetings/trainings. Facilitates discharge planning and implementation. Conducts group therapy sessions with techniques appropriate to the ages of the patients served. Provides intern and/or staff supervision, as requested by the Director of Social Work or Deputy Chief Operating Officer of Clinical Operations. Required Skills/Abilities: Must have the ability to function optimally in a stressful environment, and the ability to remain calm in emotionally charged situations. Experience with Dialectical Behavior Therapy (DBT) and other evidence-based therapeutic modalities. Proficient in group work and family systems approaches. Strong experience in comprehensive assessment within a mental health context. Education and Experience: Master’s degree in social work, Mental Health Counseling or related field. Licensed as a Licensed Masters Social Worker (LMSW), Clinical Social Worker (LCSW), Licensed Professional Counselor Associate (LPC-A) in the State of Connecticut. Experience working with addiction and/or a psychiatric patient population required. CPR BLS Certification required Nonviolent Crisis Prevention Certification required Previous experience in a mental health setting. Familiarity with electronic medical records preferred. Physical Requirements: Ability to sit for prolonged periods at a desk and working on a computer.
