The Inpatient Clinical Pharmacist oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens. This role involves providing patient and caregiver counseling on medication therapy, adverse effects, compliance, appropriate use, handling, and administration. The pharmacist designs, recommends, monitors, and evaluates patient-specific pharmacotherapeutic regimens, ensuring implementation aligns with policies and procedures. Key responsibilities also include reviewing medication orders, participating in medical emergencies, supporting medication safety and quality improvement, and performing quality assurance of the medication use system. The position requires collaboration with the healthcare team on formulary adherence and medication-use processes, assisting with non-formulary and investigational medications, and ensuring compliance with all regulations. Additionally, the pharmacist prepares and dispenses medications, serves as a preceptor for students and residents, mentors pharmacy teammates, and directs the work of pharmacy support staff. The role demands demonstrating knowledge and skills appropriate to the age of patients served, understanding growth and development principles, and interpreting age-specific patient requirements.
