About The Position

We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team and play a crucial role in ensuring safe, effective, and individualized medication therapy for our patients. Location: Bethesda North Hospital at 10500 Montgomery Road, Cincinnati, OH 45242 Work Hours: Full-time, 80 hours bi-weekly Shift varies Weekend and holiday rotation (per schedule) Minimum Job Requirements: Bachelor's degree in pharmacy Active pharmacist license in the state of OH Prescription formula and mathematics for calculating dosage and quantity Working knowledge of all Ohio Board of Pharmacy regulations 1-2 years of experience as a Pharmacist is required. Inpatient or infusion pharmacy experience preferred. Job Summary: As a Clinical Pharmacist, you will collaborate closely with physicians, nurses, and other healthcare professionals to optimize medication therapy and improve patient outcomes. You’ll provide critical expertise in drug therapy management, medication reconciliation, and patient education, ensuring that our patients receive the highest standard of care. The Clinical Pharmacist collaborates with other health care providers to ensure the appropriateness of all elements of pharmaceutical care and independently operates within policies and procedures to manage the department’s clinical and consultative services. The Clinical Pharmacist supervises the work of supportive team members to ensure accuracy and efficiency of distributive services.

Requirements

  • Bachelor's degree in pharmacy
  • Active pharmacist license in the state of OH
  • Prescription formula and mathematics for calculating dosage and quantity
  • Working knowledge of all Ohio Board of Pharmacy regulations
  • 1-2 years of experience as a Pharmacist is required.

Responsibilities

  • Patient Care: Contributes to direct patient care through careful review of patient information, assessment, and provision of appropriate pharmaceutical recommendations.
  • Promotes the correct and appropriate use of medications by maximizing clinical effect, using the most effective treatment, minimizing the risk of treatment-induced adverse events, and minimizing the expenditures for pharmacological treatments.
  • Collaboration: Establishes effective collaborative working relationships and open and active communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians.
  • Medication Management: Ensures the 7 patient “rights” are respected: right patient, right dose, right route, right time, right drug with the right information and documentation.
  • Education & Training: Provides education/counseling to patients about drug therapies, and provides concise, timely responses.
  • Assists in orientation and training of new hires, and precepts pharmacy students and residents.
  • Quality Improvement: Undertakes projects that will improve the quality of patient care and promote cost-effective use of medications.
  • Communicates with the Pharmacy Buyers to ensure supply matches' demand.
  • Ensures complete, accurate, and timely documentation.
  • Professional Development: Takes the lead in identifying opportunities and solutions in pharmacy process improvements.
  • Assumes personal responsibility for professional development, particularly in the area of ongoing clinical education.
  • Participates in relevant committees.
