About The Position

This position provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. The role serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. The position will be trained on both clinical and operational workflows and may fill gaps on decentralized clinical pharmacy shifts or a central pharmacy shifts. PRNs are needed to help fill in on weekends sometimes and candidates should be open to overnights.

Requirements

  • Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacy.
  • No experience required.
  • Basic Life Support Healthcare Provider (BLS HCP) required within 90 days of employment.
  • State of Work Location: Illinois Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR).
  • State of Work Location: Missouri Pharmacist - Missouri Division of Professional Registration.
  • State of Work Location: Oklahoma Pharmacist - Oklahoma State Board of Pharmacy.
  • State of Work Location: Wisconsin Pharmacist - Wisconsin Department of Safety and Professional Services.

Responsibilities

  • Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable.
  • Documents medication related plans and interventions in electronic medical record.
  • Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
  • Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration.
  • Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
  • Supervises pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery system.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.
