About The Position

Delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy. Exercises sound judgment, and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. Designs, implements, and manages the Antimicrobial Stewardship Program in alignment with local and national guidelines. Advocates for appropriate antimicrobial use and influences prescribing practices to improve patient outcomes and reduce antimicrobial resistance. Tracks and analyzes antimicrobial usage trends and reports finding to stakeholders. Uses data to develop and guide policy and practice improvements. Educates healthcare professionals, patients, and the public on antimicrobial stewardship principles. Provides ongoing training to pharmacy staff and other clinicians. Collaborates with Infection Prevention teams to reduce healthcare associated infections. Participates in quality improvement initiatives. Participates in multidisciplinary rounds and provides direct patient care consultations. Serves as a local representative for the Sutter Health System Antimicrobial Stewardship Program providing collaboration on shared goals and projects. Precepts PGY1 Pharmacy residents and APPE students.

Requirements

  • PHARMD - Graduate of an accredited pharmacy school.
  • Current registration or Registered Pharmacist within 120 days.
  • ACLS - Advanced Cardiac Life Support certification.
  • 3 years recent relevant experience.
  • Ability to influence physician prescribing at the service-line level.
  • Demonstrated skills in managing projects and initiatives.
  • Knowledge of medical terminology, pharmaceutical names, calculations, and laws.
  • Basic working knowledge of legal requirements and accreditation standards.
  • Written and verbal communication skills to explain sensitive information.
  • Well-developed time management and organizational skills.
  • General knowledge of computer applications and electronic health records.
